• Duration: 13 weeks\n
• Shifts per Week: 5 (Monday - Friday)\n
• Shift Duration: 8-hour day shifts (8:00 AM - 5:00 PM)\n
• Hours per Week: 40\n
• Estimated Weekly Pay: $1,755 - $1,950\n
• No weekends or on-call required \n

\n\nRegistered Nurse Qualifications\n

\n
• Active Wisconsin or Compact RN License – required\n
• Minimum 12 months of recent Home Health RN experience within the last 3 years\n
• Proficiency with HCHB (Homecare Homebase) – required\n
• Experience with OASIS documentation – required\n
• BLS certification – required (must be current at time of application)\n

• Provide skilled nursing care to patients in their homes throughout Neenah, Appleton, and the Fox River Valley\n
• Conduct comprehensive patient assessments and develop individualized care plans\n
• Document patient care accurately using HCHB and OASIS systems\n
• Collaborate with interdisciplinary healthcare teams to achieve optimal patient outcomes\n
• Meet productivity expectations of 30 points per week\n
• Deliver compassionate, patient-centered care while maintaining high standards of clinical excellence\n
