Job Details

• The Clinical Nurse I role provides direct patient care in accordance with the Nurse Practice Act to assigned patients in the Home Health and/or Hospice setting.
• The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care.
• Responsibilities include physical assessments, IV, infusion, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned.
• Utilizes the nursing process based on evidence-based best practices to meet patient care needs.
• Evaluates and adjusts the nursing care process as necessary with consultation as needed and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions.
• Performs other duties as assigned.

Job Requirements

• Requires at a minimum of two (2) years of acute clinical nursing experience.
• Clinical home health care experience is required for Home Health.
• Hospice requires at least two (2) years of acute clinical nursing experience or home health experience.
• Completion of an Associate's Degree in Nursing or higher education is required.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
