POSITION PURPOSE: The BPSR Psychiatric Prescriber supervises professional and assigned support staff and provides professional nursing care to mentally ill and emotionally disturbed clients on an outpatient basis.

REPORTING RELATIONSHIP: BPSR Psychiatric Prescriber reports to Lead Registered Charge Nurse.

PROGRAM SUMMARY FOR HEALING OAKS HYBRID OUTPATIENT CLINIC INCLUDING URGENT WALK-IN, ADMINISTRATION, AND MOBILE OUTREACH

The Healing Oaks Hybrid Outpatient Clinic (HOC), a Short-Doyle Medi-Cal certified (SD/MC) and Drug Medi-Cal (DMC-ODS) Program, that will provide outpatient psychiatric and behavioral health services to individuals ages 18+ with severe mental illnesses (SMI), including co-occurring disorders (COD), in the Central Region of San Diego of zip-code 92114. As a hybrid outpatient and Medication Assisted Treatment (MAT), HOC will provide evidence-based services to include psycho-social rehabilitation practices with a strengths-based emphasis that include psychosocial rehabilitation, psychiatric medication management, individual and group counseling, employment support, peer support, urgent walk-in and assessment, mobile outreach, case management, MAT, ambulatory withdrawal management (WM), recovery services, and both outpatient (ODT) and intensive outpatient (IOT) substance use treatment.These services have been designed to help residents in San Diego County with specialized care for transitional-aged youth (TAY), perinatal and parenting women and their children, justice involved individuals, and individuals with co-occurring disorder to include severe mental illness.Through these services, HOC will help residents of the community increase their functionality, live more fulfilling lives, and transition into lower levels of care and ultimately achieve a sustained recovery from substance use and mental illness. HOC’s services also include specialized programs for TAY, Older Adults, and individuals with COD that are person-centered, individualized, clinically effective, outcomes-driven, culturally responsive, gender-specific, trauma-informed, developmentally appropriate, and inclusive of Evidence base (EB) treatment, rehabilitation, and recovery service approaches.

ESSENTIAL RESPONSIBILITIES AND DUTIES: Analyzes and evaluates client screening, assessment, and plan to diagnose the nature and extent of mental disorder and/or substance use disorder and refer to appropriate treatment program(s); Prescribes, directs, and administers psychotherapeutic or medications to treat mental, emotional, or behavioral disorders; Provides psychiatric consultation, and medication management services, and education to psychiatric clients; Completes an initial psychiatric evaluation, including an assessment for BPSR in a culturally appropriate atmosphere of healing; Prescribes psychotropic and related medications based on clinical indicators of a client’s status, including results of diagnostic and lab tests as appropriate to treat symptoms of psychiatric disorders and improve functional health status; administers/monitors the use of and notes reactions to psychotropic drugs and other medications; Monitors the physiological and psychological effects (and side-effects) of the medications prescribed to program clients, on a regular basis, and make appropriate interventions and modifications when necessary; Conducts consultations with clients’ family members, case managers, and/or caregivers; Collaborates with physicians, nurse practitioners, psychiatric nurses, psychologists, social workers and other professionals to discuss treatment plans and progress; Performs timely, accurate, and concise documentation of client care; Assesses clients in crisis situations and facilitates prompt resolution via administering medication, crisis counseling, and/or or referral to other services; Consults and collaborate with members of the treatment team to assist clients in obtaining, and making appropriate referrals for necessary or requested services physicians, nurses, social workers and counselors and other health professionals to assist clients in obtaining, and makes appropriate referrals for, necessary or requested services; Participates in shared program responsibilities, such as utilization and transition reviews; Completes a ll other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of clinical assessment of clients with severe mental illness; BPSR guidelines, principles of care and recovery treatment modalities; Brief solution-focused recovery mental health services model; Quality assurance principles and practices; Local community and mental health and primary care resources available to assist the severe mentally ill; Diagnostic categories (DSM V-TR); Appropriately identify, assess, diagnose, and effect appropriate treatment plans utilizing community resources and psycho-social treatment modalities in meeting the needs of clients; Interview for the purposes of acquiring diagnostic information and effecting treatment; Supervision and training for clinical and nursing staff; Evidence-based practices and issues pertaining to psychosocial treatments for SUD, Motivational Interviewing, and Relapse Prevention. Experience working with a culturally diverse population and knowledge of cultural issues, values, & beliefs of the target population of adults w/ SMI, homelessness, co-occurring disorders, and criminogenic risks and needs, relevant to providing appropriate BPSR program services; Clinical assessment of clients with severe mental illness, counseling theory, and clinical care coordination principles, and best practices, cultural and socio-economic issues relevant to providing appropriate rehabilitation and recovery services to special population groups, including Latino, African American, and LGBTQIA+ communities, human behavior and the social environment; Exercise sound, expert independent judgment within general policy guidelines; Communicate clearly and concisely; operate a computer using word processing, spreadsheet, and database software applications; Operate standard office equipment.

EXPERIENCE AND TRAINING: A medical degree with a valid physician medical license and at least five (5) years’ experience working in a psychiatric or outpatient facility . Must be able to provide medical supervision to Nurse Practitioners as needed.

LICENSES, CERTIFICATIONS & SPECIAL REQUIREMENTS: Must have a valid and active physician medical license issued by the State of California. A Valid Driver’s License with current California automobile insurance; A Valid Cardiopulmonary Resuscitation (CPR) Certification; A Valid Tuberculosis Blood (TB) test. Some positions require transportation of clients in personal vehicle.
