Job Summary:
The Provisionally Licensed Mental Health Practitioner (PLMHP) offers direct mental and behavioral health services to patients of Midtown Health Center. Position also involves assessment of the patient to determine needs, referral, and related activities to help the patient obtain needed services, monitoring, follow-up, and evaluation.
Supervisory Responsibilities:
None.
Duties/Responsibilities:
- Provides interventions as needed to MHC’s patients within area of expertise and limits of credentials.
- Provides therapeutic interventions, counseling and instruction about mental health needs.
- Completes appropriate assessments of patients to establish a foundation for treatment plans.
- Works with patients on a day-to-day basis using professional judgment and discretion to assure that patients obtain needed services and care.
- Assists in the development, implementation, and revision of individual or family treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals.
- Identifies and addresses patient’s risk factors and/or obstacles to care.
- Identifies patient’s needs, current services, and available resources, and then connects the patient to services and resources to meet the patient’s needs and goals of treatment.
- Communicates the care preferences of patients, serving as their advocate, and verifying that inventions meet the patient’s needs and goals of treatment.
- Serves as a liaison between the patient and primary care provider to resolve care concerns as they arise and collaborates with the primary care provider to assure care is provided in an integrated manner.
- Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up with the focus on attaining treatment goals.
- Keeps abreast of new information and materials by reviewing clearinghouse information, newsletters, and periodicals.
- Researches and designs educational materials as indicated by best practices and patients’ needs to assist in developing and obtaining personal health goals.
- Performs other duties as assigned.
Duties 1 through 12 are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.
Knowledge, Skills, and Abilities:
- Ability to efficiently carry out Midtown Health’s Center mission statement and comply with its policies and procedures.
- Ability to establish and maintain effective working relationships with patients, co-workers, and the public and to be able to demonstrate cultural awareness and sensitivity.
- Must be able to maintain strict confidentiality of sensitive information in accordance to HIPAA regulations.
- Ability to comply with all occupational safety and health standards and all rules, regulations, and orders issued under the Occupational Safety and Health Act (OSHA).
- Ability to utilize critical thinking and problem solving skills to address clinic’s needs.
- Ability to interpret oral and written instructions and do appropriate follow-up.
- Ability to work independently and as a team member.
- Must have exceptional time management and organizational skills.
- Ability to constructively participate in quality improvement efforts.
- Ability to actively participate in marketing and outreach activities when requested and emergency response events when required.
- Bilingual (English/Spanish) verbal and written skills highly desired.
- Must have knowledge of current and best practices of mental and behavioral health.
- Must consistently exercise discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
- Must have excellent interpersonal and communication skills in one-on-one and group situations and using the telephone.
- Must have knowledge of community resources and have the ability to assess need for referral.
Education and Experience, Licensure and Certification:
- Master’s degree in social work, human services, psychology, sociology, or other related field from an accredited college/university required.
- Training in crisis intervention and case management preferred.
- Experience in community-based behavioral health care and case management preferred.
- Must have a current, Provisionally Licensed Mental Health Practitioner license in the State of Nebraska.
- Must have a valid driver’s license and the ability to utilize own transportation for work purposes.

- Maintain physical condition appropriate to the performance of assigned duties and responsibilities which may include sitting or standing for extended periods of time.

- Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard.
- Vision and hearing within normal parameters to perform job requirements.

- Ability to work varied and extended hour scheduling as requested including early mornings, evenings, and occasional weekends.

- The position works with mentally ill individuals where a high degree of stress is possible.
