GENERAL SUMMARY AND PURPOSE:
Provides advocacy, education, assessments and interventions to patients and families in the Outpatient Cardiovascular Pulmonary Rehabilitation setting to achieve an optimal level of care and patient participation. Coordinates with medical staff and outside organizations to establish specific treatment plans and program participation planning to achieve quality clinical outcomes.
SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE :
Licensed Clinical Social Worker (LCSW) required in Idaho.
Licensed as a LCSW in Oregon within 12 months of hire.
Minimum of one year of post-license experience in behavioral health or integrated care.
Working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions preferred.
ESSENTIAL FUNCTIONS :
Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions.
Provides interventions for patients, families, and caregivers as appropriate. Interventions may include but are not limited to Social Screener, upfront clinic, mental, behavioral health screener to provide the support to all parties in an advisory role for the Cardiovascular Pulmonary Rehabilitation (CVPR) program efficacy. Advisory role includes grief counseling, supportive therapy, crisis intervention, deceleration skills, PTSD counseling, psychiatric placement/referral, alcohol/drug assessment and referral, and referral to alternative community resources.
Participates in care coordination activities for those patients identified as needing social work and/or complex case management interventions.
Works collaboratively with healthcare team members to efficiently allocate resources.
Accurately documents assessments, plans, and interventions in the EMR.
Responsible for coordination of care to mental health and/or substance abuse facilities.
Responsible for building program retention and participation. This includes ensuring the patients are able to complete the program within the prescribed time.
Responsible for following established patient choice policy.
Responsible for coordinating with physicians and other legal entities relating to, but not limited to, guardianships, involuntary psychiatric holds, power of attorney, and advanced directives.
Collaborates with physicians and CVPR staff through formal and/or informal multidisciplinary case conferences to meet the emotional, mental, physical and spiritual needs of patients and/or their families/caregivers.
Responsible for Pyscho/Social initial exam portion of the patient while assessing the patient for fidelity to the program.
Actively participates in department staff meetings and other teams or committees as required.
Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, Geriatric as applicable).
Adheres to the National association of Social Work (NASW) Code of Ethics and the State Licensure Board of ethics.
