LHH Recruitment Solutions is seeking a dedicated and compassionate Bilingual Case Manager to join our team in Los Angeles. This role involves providing essential support services to clients, helping them access resources, and empowering them to achieve their personal and professional goals. The ideal candidate will be fluent in English and another language, possess strong interpersonal skills, and have a passion for making a positive impact in the community.

Key Responsibilities:

• Conduct client assessments to identify needs and develop individualized case plans.

• Serve as a liaison between clients and community resources, including healthcare, housing, and employment services.

• Facilitate access to primary care, mental health services, and other critical resources.

• Maintain accurate and up-to-date client records in compliance with company policies and regulations.

• Collaborate with team members to ensure comprehensive support for clients.

• Provide culturally sensitive and language-appropriate services to clients.

• Monitor client progress and adjust case plans as needed.

Qualifications:

• Fluency in English and another language (Spanish preferred).

• Bachelor's degree in Social Work, Psychology, or a related field (or equivalent experience).

• Previous experience in case management or a similar role, preferably in a social services or healthcare setting.

Pay Details: $22.00 to $26.00 per hour
