Job Description & RequirementsRequirements
- Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience
- EPIC CHARTING experience is REQUIRED
- Experience must include at least 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge
- Certification Requirements : BLS
