LanceSoft is seeking a travel nurse RN Care Manager for a travel nursing job in Bridgeport, Connecticut.
Neuro unit (stroke, s/p crani, etc) with gen medical and complex long LOS patients
Years of Exp preferred: minimum 3 years, 5 years preferred Must have EPIC experience, ability to cross-collaborate with SW, therapy, and medical team, as well as assigned Case Manager Assistant (who will manage SNF referrals and transportation), as well as consulting with Complex Case Manager (on complex cases and long LOS patients). Must be able to multi-task. Progression rounds daily (at 10am), should have intimate knowledge of existing patients and contribute to the discussion during rounds. Shift: M-F 8a-4:30p; 40 hours; May need to work Sat/Sun for weekend coverage, would take a day off during the week to stay within 40 hours. #HCRR
