As a direct assistant to Social Services in our Skilled Nursing setting, this team member will provide counseling and support to residents, coordinate resident care conferences, assure regulatory compliance, accurate documentation and an excellent resident and family experience. Keep staff informed of resident's status and condition to utilize appropriate skills, intervention assistance and support from trained and skilled staff at Seabury Current Openings: Part-Time: 24 hrs/wk various weekdays from 8:30am-5:00pm. Employee discount in the on-site Bistro Free secure parking Highly Vaccinated Campus Position Requirements: Education : Minimum of Bachelor's Degree in Social Work or related field Experience : 2+ years long-term care experience, required. Working knowledge of PASSR, MDS, BIMS AND PHQ9 screenings. Skills and Competencies: Ability to read, write, speak, and understand the English language clearly. Must be able to relate to the elderly in a dignified manner and build a relationship with residents that are positive and supportive. Must be extremely knowledgeable and competent in all processes of SNF admissions and discharge procedures - documentation and compliance requirements.
