Job Description

Village Green Memory Care Community is seeking a dedicated and compassionate Remote Health and Wellness Coach to join our team. As a leading memory care community, we are dedicated to providing exceptional care and support for individuals with Alzheimer's and other forms of dementia. The Health and Wellness Coach will play a vital role in promoting the well-being and overall health of our residents.

Responsibilities

• Develop and implement individualized wellness plans for residents based on their specific needs and goals
• Conduct health assessments to determine residents' current health status and identify areas for improvement
• Provide education and guidance to residents on nutrition, exercise, and activities that promote overall well-being
• Coordinate and lead group exercise classes and wellness programs
• Collaborate with the interdisciplinary team to ensure that residents' needs are met and their goals are being addressed
• Monitor residents' progress and make necessary adjustments to their wellness plans
• Provide emotional support and encouragement to residents and their families
