Job Title: Care Coordinator II
Location: Remote (Preferred: Tampa Bay, FL area)
Pay Rate: $25/hour
Contract Duration: 6 months (potential for extension or conversion to full-time)
Shift: Monday – Friday, 9:00 a.m. – 6:00 p.m. EST

Position Overview:
The Care Coordinator II supports care management activities to ensure continuity of care and member satisfaction. This role involves telephonic outreach, coordination with healthcare providers, and documentation of member care plans. Occasional in-person meetings in Tampa Bay, FL, may be required.

Key Responsibilities:

• Conduct outbound calls to members to discuss care plans, follow-up questions, and provide ongoing education.
• Coordinate care activities with healthcare providers, community partners, and members/caregivers.
• Address member and provider inquiries related to care plans.
• Communicate with care managers and practitioners to ensure continuity of care.
• Perform service assessments/screenings as needed and maintain accurate member records.
• Identify needs and make referrals to care managers or community-based organizations.
• Educate members about benefits and resources available.
• Document daily activity reports and maintain compliance with policies and regulatory guidelines.

Performance Metrics:

• Complete 50-75 calls daily.
• Submit daily production reports through Outlook.

Required Skills and Experience:

• 2+ years of experience in healthcare or managed care (e.g., call center, doctor’s office).
• Comfortable with outbound/inbound calling and multitasking (e.g., making calls while taking notes).
• Experience with Microsoft Office Suite (Excel, Outlook, Word, TEAMS).
• Adaptability to change.

Preferred Qualifications:

• Bilingual in Spanish or Creole.
• Pediatric care experience.
• Previous work-from-home experience.

Education Requirements:

• High School Diploma or GED required.
